Squamous lesions: Atypical squamous cells (ASC):Atypical squamous cells of undetermined significance (ASC-US) Atypical squamous cells cannot exclude high-grade squamous intraepithelial lesion (ASC-H) Cells that display abnormalities more marked than simple reactive changes but qualitatively or quantitatively do not fulfill the criteria for a squamous intraepithelial lesion; in some cases, these lesions are associated with cervical intraepithelial neoplasia (CIN). Cells that alone (or in combination with low-grade squamous intraepithelial lesions [LSIL]) share cytologic features with high-grade squamous intraepithelial lesions (HSIL). Cytologies termed ASC-H can reflect the presence of true high-grade squamous intraepithelial lesion and other entities that mimic such lesions. Low-grade squamous intraepithelial lesions (LSIL) Lesions associated with human papillomavirus (HPV) infection. These tend to be associated with transient changes that regress over time. High-grade squamous intraepithelial lesions (HSIL) Lesions associated with high-risk types of HPV and that have a high risk of a CIN 2,3 outcome, persistence, or progression to cancer. Glandular lesions: Atypical glandular cells (AGC) Either endocervical (AEC), endometrial, or not otherwise specified (NOS) is noted as a subcategory. This replaces the previous term "atypical glandular cells of undetermined significance (AGUS)." Atypical glandular cells, favor neoplastic Endocervical is the only subcategory. This designation is for specimens that show features suggestive of, but not sufficient for, an interpretation of adenocarcinoma. Endocervical adenocarcinoma in situ (AIS) The normal glandular architecture of the endocervical glands may be preserved, but the glands are lined by atypical columnar epithelial cells similar to those of invasive cervical adenocarcinoma but without stromal invasion.References: Solomon D, Davey D, Kurman R, et al.
